FPV for my GoPro Hero3+? What do I need?

What do I need to send the video signal from my GoPro Hero3+ to a monitor or goggles and fly FPV? I think I need a transmitter, receiver and a cable from the transmitter to the camera. I'm looking to use this on my Blade 350QX. Do they make a system and app that will play back video on an iphone or ipad? Any help would be greatly appreciated.

Update: After a few hours of googling and youtubing I've figured it out.
Video Transmitter = Mounted on RC aircraft powered by flight battery or additional battery.
Camera = GoPro Hero 3+ Silver connected to video tramitter via USB cable adapter.
Video Receiver or Goggles = Receiver on the ground powered by battery and RCA out to monitor of choice like a small 7-10" LCD monitor.or my 47" LCD HDTV under my back patio - He He) or Goggles powered by battery.

I'm glad I will be able to utilize my Gopro & gimbal and not have to add an additional camera. This gonna be fun!

This is what I ordered for $230 from readymaderc.com. I'm going to power the video transmitter through the 3 cell flight battery balance plug and the video receiver and monitor ground station with another 3 cell battery.

Overnight shipping from ReadyMadeRC.com and I am a FPV first timer in the air. I made 3 flights with it yesterday evening. See the pictures below for the way I set things up. I connected the video transmitter on the 350QX to the 3 cell balance plug on the flight battery. The GoPro3+ USB cable connected to video transmitter input which also charges the camera battery at 5V. I routed the wires through the factory sloted tall landing gear skids. I opted to route the transmitter power cable and telemetry cables on the opposite side from the compass wires to prevent interfearence. I cut the 120V wall plug from the LCD monitor and ccombined it with the video receiver power and soldered a EC3 plug to connect to a standard 3 cell battery. I like the clean looks of the 350QX and want keep the add-ons descreately as possible and wiring clutter to a minimum to retain the factory looks. There is a cavity in the nose of the 350QX body. I may try to fit the telemetry module in there and run the attenna external. I may also paint the video transmitter white to match. I am also waiting on delivery of the ebay simplified stand alone 2 axis gimbal. Thanks to post found hereon FliteTest and YouTube for steering me in the right direction.

It looks like you're using 5.8 gHz. It's highly recommended to use circular polarized antennas for this frequency. There's several to choose from. I like the clover leaf style, they're small and compact.

I'd also look into building some sort of "ground station". Mostly just some sort of case to carry your screen, and extra gear in. I have converted a small cooler into my ground station.

Eventually you might want to buy a set of video goggles. I have the Fat Shark Predator V2's. They're great for me at this time, but there's lots of options out there.

You'll want some way to protect your GoPro. I bought the Layer Lens from GetFPV.com. But there's other options out there.
